The Hospitalist/Nocturnist will facilitate transfers from outside facilities, admit appropriate patients from VA clinics and emergency department, and be available for any acute clinical issues for patients already admitted to Hospital Medicine teams during their tours of duty.
Administrative Duties:
- Leadership responsibilities: Participate in staff meetings, hospital committees, and the peer review process. Demonstrate effective communication to staff and leadership.
- Human Resources (HR): Maintain VA training requirements, complete documentation in a timely manner and maintain credentials required for privileging and re-privileging.
- Staff Management: Participate in the Focused Professional Performance Evaluation (FPPE)/On going Professional Performance Evaluation (OPPE) process. Adhere to employee health requirements. Participate in the peer review process.
- Time/Leave Management: Adhere to tour of duty, log duty hours appropriately and request leave in advance.
- Clinical Management including indirect pati ent care: Manage encounters and consults appropriately, document in a timely and appropriate manner. oversee relevant performance measures, and participate in quality improvement and process improvement projects.
- The physician may be asked in future to round on patients at home, as part of hospital in home program.
Clinical Duties:
-The Medical Service Noctur in future to round on patients at home, as part of nist is a hospitalist responsible for providing care to hospitalized adult patients, identifying health problems, and designing diagnostic and therapeutic plans to meet these needs during overnight/weekend hours. Duty hours are subject to change based on t he needs of patient care. Specific duties include the following:
- Assess patients for admission and disposition.
- Admit patients to Hospital Medicine teams, either independently or working with house staff providers or mid level providers .
- Staff patients requiring Medicine consultation if urgently needed (for example, to guide surgical treatment.)
- Being available to staff procedures done during the tour of duty.
- Being available for acute medical issues that arise on any patient admitted to the Hospital Medicine service.
- Hospitalists are also responsible for managing codes and are code team leaders during non-administrative hours.
- Completing educational evaluations for the night float house staff as required by the Internal Medicine Residency Program. Adherence to VA policies and procedures regarding documentation, patient safety, and utilization. Independently assess patients for admission and disposition when dictated by house staff schedule and regulations.

Work Schedule: This position is minimum 6 months nighttime rotations, and rest of the time will be daytime rotations. The nighttime typical tour of duty (TOD) is compressed from Monday to Sunday 7:00 PM till 7:00 AM. Daytime compressed TOD is Monday to Sunday 7:00 AM till 7:00 PM. The incumbent may be asked to work 8:00 AM to 4:30 PM as needed. We also have a noon till midnight TOD Monday to Sunday and some daytime duties may include that TOD. In the beginning the incumbent will also be asked to work daytime shifts as to get acclimated with hospitalist workflow. These shifts usually are from 8:00 AM till 4:30 PM and usually a period of two months or more may be needed or required.

The staff optometrist at the VANTHCS is credentialed and privileged through the chief of staff in accordance with the medical staff bylaws. Optometry is organizationally part of the surgical care line and individual performance evaluations will be done by the chief of the ophthalmology section. The staff optometrist is a licensed independent practitioner responsible for patient care activities in the ophthalmology clinic. Planning, coordinating and exercising professional judgement in initiating and implementing clinical and administrative decisions are integral components of the position.
Clinical Functions:
Exercises independent professional judgement in diagnosing the full range of ophthalmic conditions as well as planning and executing the necessary treatments for patients who have illnesses within the scope of optometry. The scope of these activities covers all phases of ambulatory care. Promotes communication between eye clinic team members, patients and their families to facilitate the medical treatment process. Consults with ophthalmologists and others, when necessary to augment the care of patients as it relates to emergent and traumatic conditions as well as ophthalmic manifestations of systemic conditions. Utilizes the general clinical privileges which include ordering diagnostic tests and writing prescriptions for medications or other items on the hospital formulary. The optometrist should also demonstrate knowledge of preventive medicine and primary care as related to optometry (e.g. complications of disorders and treatments, side effects of medications used). Independently exercises creativity in solving problems without direct supervisory guidance to meet the varying needs of patients while under care in the eye clinic and assures that a high degree of quality patient care is provided at all times.
Administrative Functions:
Maintains accurate and timely patient medical record entries in Computerized Patient Record System (CPRS). Also complies with all VHA Privacy Policy (HIPPA) requirements. The incumbent will attend regularly scheduled section, surgical service and medical staff meetings. Performs administrative duties at the direction of the ophthalmology/optometry section chief. Follows accepted infection control and safety procedures as well as other station policy procedures.
Research Functions:
Participates, where appropriate, in research in basic visual science and clinical optometry including epidemiological studies, clinical trials and evaluation of alternative health care delivery systems to enhance patient care.
Education Functions:
Participates fully in educational development through staff and section meetings and assumes responsibility for professional development by attending relevant training and regularly reviewing the literature to improve clinical competencies and meet continuing professional education requirements to maintain current competence and licensure.

Required Experience:
