The Nursing Assistant is directly responsible to the Registered Nurse (RN) for providing direct care within his/her scope of practice. The RN establishes priorities for care. The Psychosocial Rehabilitation and Recovery Center (PRRC) serves Veterans with a range of psychiatric conditions and specializes in Serious Mental Illness (SMI) disorders. This social worker independently assesses the psychosocial functioning and needs of Veterans and their family members, identifying the Veteran's strengths, weaknesses, coping skills, and psychosocial acuity. The Psychosocial Rehabilitation and Recovery Center (PRRC) serves Veterans with a range of psychiatric conditions and specializes in Serious Mental Illness (SMI) disorders. This social worker independently assesses the psychosocial functioning and needs of Veterans and their family members, identifying the Veteran's strengths, weaknesses, coping skills, and psychosocial acuity. In collaboration with the Veteran, family/caregiver, and interdisciplinary treatment team, the social worker facilitates the delivery of health care services.

The social worker provides independent interventions with Veterans and their families who are experiencing a wide range of complicated medical, behavioral health, financial, legal, and psychosocial problems and provide a range of interventions and treatment modalities which may include individual, group, and/or family counseling or psychotherapy.

Major duties and responsibilities include:

• Link the Veteran with services, resources, and opportunities to maximize independence, health, and well-being.
• Conduct timely assessment of at-risk Veterans in crisis to identify immediate needs, evaluate risk, and initiate safety plan as appropriate.
• Provide case management and care coordination to facilitate appropriate delivery of health care services, incorporate complex multiple causation in differential diagnosis and treatment of Veterans, and make psychosocial and psychiatric diagnoses within approved clinical privileges or scope of practice.

Duties include, but are not limited to:

• Provide a full range of services within the Social Work Scope of Practice based on Veteran needs, acuity level, and preferences for care. Services may include, but are not limited to, the following: case management; individual, family, and group therapy; discharge planning; resource linkage; and care coordination.
• Provide consultation and psychoeducation to Veterans and Veteran families on topics including community resources, Advance Directives, VA benefits, budgeting, activities of daily living, and recovery goals.
• Act as a consultant to other team/staff members and take a lead clinical role in case consultation.
• Fulfill ethical responsibilities to Veterans, colleagues and VA in accordance with NASW Code of Ethics, VHA Code of Ethics, and local ethics policies. The Nursing Assistant (NA) is a member of the Acute Care Team, providing assistance to the providers, nursing leadership team, registered nurses, and other team members. The NA demonstrates the knowledge and ability to provide care to Veteran patients within the Acute Care Service Line. This service line cares for patients admitted to an inpatient medical-surgical inpatient hospital unit (2 West, 5 East or 6 West) at VA Puget Sound, Patient Care Service Line.

Duties will vary based on grade. At the GS-5 level, duties will include, but are not limited to:

• Participates in activity and actions to improve the quality of care for veteran patients to include fall and pressure sore prevention.
