As the Regional Advisor, Sector Support & Development (SSD), you will play a pivotal role in preparing Commonwealth Home Support Program (CHSP) organizations within the Eastern Metropolitan region, as well as on a national scale, for the evolving landscape of aged care reforms. This position involves fostering collaboration, enhancing provider readiness, and promoting programs underpinned by diversity, wellness, reablement, and restorative care principles. You will build and nurture strategic partnerships within the CHSP network and the broader aged care system, facilitating communication and sharing best practices. About you

We are looking for a Support Worker to work casually across our Home Care Packages program.
Home Care Packages responsibilities:

• Deliver high-quality individualised support and assist our older adult clients, supporting them to maintain and develop their independence so that they can lead enriching and fulfilling lives.
• Follow and implement individual service plans, healthcare, and positive behaviour support plans.
• Provide companionship, conversation, and emotional support and support with personal care in the client's home.
• Assisting with transportation to attend appointments.

This position will be a part of our Social Activity Group with the key focus being to work alongside older people whilst in the supported social activity group to enable their identified needs and goals to be fulfilled, to make decisions and choices about their own lives, to facilitate their access and participation in their community, and to remain living in their homes independently.

Although a typical day might be hard to come across, you will regularly be engaging with the clients in groups on outings or in our centre as they challenge their minds in a variety of activities or even at a simple gathering sharing stories.

Social Activity Group responsibilities:

• Assist and work alongside older people whilst in a supported group to enable their identified needs and goals to be fulfilled.
• Encourage and support people to actively participate in planning and feedback forums when making decisions about the service they receive, and ensure activities are responsive to this feedback.

• Ensuring cultural sensitivity in communication and interaction with families.

We are looking for someone who enjoys working with children 2-7 years and their parents, has excellent IT skills and can support clinicians in both administrative and practical ways. Your experience or education in early childhood development will help you thrive in this role.

Your Qualifications and Skills

• Understanding of children’s developmental stages
• Experience working with young children within a group setting
• Strong IT and administrative skills
• First aid and CPR certification
