Duties - Professional To take history, examine and record all findings of all patients admitted under the Consultants care. Carry out and arrange all relevant investigations. The post holder: Is responsible to the Consultants and Registrar for the execution of their professional duties. They shall ensure that patients admitted to the ward undergo a medical assessment and ensure that a robust and appropriate management plan is in place, with the support of their team. They will attend 2 consultant led ward rounds per week and will undertake daily ward rounds (either supported by a SpR or independently led) They will, together with the team, undertake ward work to ensure that investigations are requested and treatment plans are completed. They will ensure that good communication is maintained with staff in all specialties in order to provide joined-up care for the patients. They will work as a valued member of the multi-disciplinary team and contribute to meetings and team decisions on patient care. They are responsible for the daily maintenance of in-patient notes, preparing discharge letters (TTAs) for immediate dispatch to the patients General Practitioner and communicate with primary care and community services as needed. They shall be expected to speak with patients and their next of kin if required and provide an update on management plans. In carrying out their duties the Doctor will always be responsible for keeping their Senior Staff informed of serious cases on the ward, deaths, and any other matter which it is deemed necessary that their attention should be drawn to. Any cases in which there is doubt must be referred to a senior member of staff. General Medicine On-call The Medical Take is staffed by 1 SpR, 2 SHO grade and an F1 grade doctor. There is an average of 25-30 patients admitted over a 24-hour period. During the day, there is a continuous consultant post take ward round providing the opportunity for immediate feedback and learning. Overnight, Consultant advice is available by telephone, and patients are reviewed on the morning post take ward round.

St Georges Hospital is a major teaching hospital in SW London. General Paediatrics:

This post will suit those wishing to gain substantial experience in a large, well supervised unit with a mixed general and subspecialist patient group. Training can be tailored to needs of the postholder but there are opportunities to experience emergency care, ambulatory care and care of complex children. Duties will involve daily assessment of the children, planning of treatment, organising and arranging investigations, and liaison with other departments and teams.

Paediatric Infectious Diseases/oncology:

You will be part of the ward team caring for all patients on Pinckney ward, the paediatric infectious diseases and oncology ward. Children admitted to Pinckney ward have a variety of infectious conditions such as tuberculosis, meningitis and HIV. You will also be involved in caring for patients with an oncological diagnosis who are admitted either for chemotherapy or febrile illness. You will have the opportunity to attend infectious diseases meeting, and attend specialty clinics.

Paediatric Surgery:

St George’s is the regional referral centre for children requiring paediatric surgery. Children are admitted electively for a range of procedures, many of these with co-morbid medical conditions and complex needs. Acutely children are also referred for emergency surgery from regional hospitals.

Paediatric Neurosciences:

St George’s is also a regional neurosurgical centre and hosts regional tertiary paediatric neurology. The teams work closely in the care of children with complex needs and provide a number of specialist treatments. St George’s is also a major trauma centre and provides care for children with the sequelae of major trauma. Duties will involve daily assessment of the children, participating in neurosurgical and neurology consultant led ward rounds, and working with the specialists to arrange investigations and treatment.

Paediatric Community Services:

The paediatric neurodevelopmental, neurodisability and epilepsy service is well established and offers a child-centred, consultant led service. It is part of the integrated child health department of St George’s Hospital and offers a comprehensive service for a wide range of neurodevelopmental problems (including ADHD and ASD), complex neurodisability, Down’s Syndrome, Cerebral Palsy and Epilepsy. The ADHD Service is a specialist Attention Deficit Hyperactivity Disorder (ADHD) service within the developmental paediatric team. The service supports parents and children through the assessment, diagnosis and management of ADHD. We take referrals of children under the age of 11 years old who are registered with a GP in the Borough of Wandsworth. We provide follow up care for children who are under our service, until they are old enough to transition to adult services.

Chelsea Children’s Hospital is currently recruiting Junior Clinical Fellows in the field of Acute/Inpatient Paediatrics and Paediatric Emergency Medicine for 6-12 months on a fixed term contract. The posts commence in August/September 2024 but there is scope for an earlier start depending on individual needs

These posts cover the paediatric in-patient and out-patient clinical care and will work within the existing paediatric team. They are ideally suited for trainees who want to gain experience in general paediatrics and specialist areas such as Paediatric High Dependency Care (PHDU), Gastroenterology and Paediatric Surgery. The rotation across different specialties will vary, to match the skills of the individuals and needs of the department.

The post is part of a 15-person full time rota providing out of hours cover to the general paediatric inpatients and emergency admissions. The out of hours team is made up of a resident Paediatric Consultant, a Paediatric Medical Registrar, a Paediatric Surgical Registrar and two junior doctors (ST1-ST3 level); this provides both an excellent level of care for children and extensive opportunities for training.

The department is a recognised RCPCH PEM training centre, being one of only five departments in the region providing specialist PEM registrar training. It also provides training to Emergency Medicine CT3s gaining their PEM experience. Recent London School of Paediatrics and GMC surveys are consistently rating the post as one of the best in London.

The unit is supported by a large inpatient paediatric unit, with most paediatric subspecialties represented on site including paediatric surgery, anaesthetics and the regional burns unit.

Although these posts are not recognised for training, they provide excellent training opportunities. The postholders are expected to keep a training portfolio, have regular assessments with an allocated educational supervisor and will be encouraged to participate in audit and quality improvement projects and demonstrate progression in the field of Paediatrics during their tenure.
