The Transition and Post Care Support Officer key responsibilities are:
? Provide specialist transition from care support that identifies and appropriately responds to the complex support needs of high-risk young people who are at risk of homelessness based on the principles of the Strengthening Families Protecting Children Framework for Practice and the core elements of the Aboriginal and Torres Strait Islander Child Placement Principle. ? Promote and facilitate involvement of the young people in goal setting and planning for their transition while supporting their connection to family, culture and community. ?
