To develop and deliver a service to improve the health and wellbeing of patients requiring assessment for heart failure, working within the heart failure team and the virtual ward. The West Suffolk Virtual Ward provides specialist led care to a variety of medical and surgical patients (adult and paediatric) across a broad range of specialties. Care is provided to them in the comfort of their own homes using an advanced personal remote monitoring platform, enabling them to safely leave hospital sooner or not come into hospital at all. The Virtual Ward team includes community nurses, specialist nurses, advanced care practitioners, consultants, therapists, healthcare assistants and a range of other health and social care staff. The postholder will be working within the heart failure service (acute and community setting) and will work closely with the Virtual Ward team providing specialist advice and support for patients with heart failure, identifying and assessing patients suitable for Virtual Ward care, reviewing patients on daily ward round calls and providing advice on treatment, escalation, and discharge. The job plan will include outpatient heart failure clinics within primary and secondary care. This post is available for Band 6 or Band 7 pending on previous experience and qualifications. If criteria for Band 7 is not met there will be support and opportunity to progress within the role. Expectations for progressing to Band 7 would be completion of Independent Nurse prescriber course, working towards masters level programme, independently supporting heart failure clinics and Virtual ward duties. The aim once working independantly between the team is to offer cover over a six day working week. Deliver the Individual Placement and Support (IPS) approach for which training will be given. Meet and support clients to understand their key skills, aspirations and goals through completing a Vocational Profile and produce an Action Plan to help them obtain and sustain competitive employment. This includes support with their job search, CV production, application forms, interview techniques and career development. Assess clients support needs related to work which may include benefits/welfare advice, disclosure of mental health symptoms etc, and provide support & guidance. Attend weekly clinical team meetings as an embedded IPS practitioner. We are seeking a patient focused pharmacist aiming to build on established clinical skills and work closely with clinicians in providing a high-quality service to patients within ESHT. We are looking for a dynamic and experienced hospital pharmacist to work with us in a portfolio style role within acute medicine division and antimicrobial use. This post will be based at Eastbourne District General Hospital, although there may be an element of cross site working at Conquest Hospital and involve evening duties. Under the supervision of a designated Lead Pharmacist, you will work closely with consultants, other medical staff, nurses, pharmacy staff and other allied healthcare professionals in ensuring that medicine use is safe, effective and evidence based across ESHT. The post holder will be involved in providing clinical pharmacy services to Acute inpatient wards and occasional support to the dispensary. This post also holds an education and training commitment, which in the past has proved successful in developing staff. Support the design, implementation and ongoing administration of key compensation and benefit arrangements.

Liaise with providers and organise employee benefits as requested

Assist with any additional HR tasks as required

Knowledge, Skills & Experience:

5 Years within a payroll role
Experience with year end reporting
HR Admin knowledge would be advantageous
Experience in manufacturing / Engineering / Production environments would be advantageous


Benefits Package:

Salary:- £33,000 - £35,000 + 5% Bonus
Hours:- 36.25hrs a week
