More detail about the role The post holder will be expected to provide best Nursing care for patients with TB, ILD, Obstructive Sleep Apnoea, Type 2 Respiratory Failure and those that require oxygen therapy. This will involve providing effective disease management for complex patients, with the aim of avoiding Hospital admission where possible and reviewing patients on the wards with a view to reduce length of stay. There is a requirement for the candidate to be able to work autonomously. The role requires good organisational and communication skills and the ability to work collaboratively with integrated care partners and as a member of a team. There is good support from Senior members of the team, Respiratory Consultants and Respiratory Advanced Clinical Practitioners and the role provides valuable opportunities to develop clinical and management skills, whilst contributing to audits and research. The Community Specialist Palliative Care Team is looking to recruit a Clinical Nurse Specialist with proven experience within specialist palliative care. The role will be varied and challenging, the service facilitates 7 day responsive visits and clinical triage as part of a Specialist Palliative Care Triage. This involves gatekeeping and initiating a clinical assessment at point of contact for all Specialist Palliative Care services.



The Post Holder will be expected to act as an autonomous practitioner providing complex Specialist Palliative Care symptom management and advice to support patients their families and carers. You will have excellent communication and interpersonal skills, promoting good collaborative team working and an ability to creatively facilitate implementation of the End-of-Life care tools such as electronic End of Life register (EPaCCs) and Advance Care Planning. You will be involved in providing education to Health and Social Care staff across a range of settings.

Specialist Palliative Care Services
The practitioner will manage an autonomous community caseload requiring specialist input for complex palliative care needs.

To act as an autonomous practitioner, providing complex specialist palliative care symptom management, education, advice, counselling, and support to community patients, their families and carers.

Provide clinical triage, adhering to the Specialist Palliative Care (SPC) referral criteria, on a rota basis to ensure patients, their families and carers with complex needs receive the most appropriate specialist input in a timely and responsive manner.

The practitioners will work with primary, secondary care and nursing home staff and the multi-disciplinary palliative care team offering specialist advice and educational input.

To contribute to the development of services according to local, regional and national directives
• To act as an initial point of contact to the Specialist Palliative Care
Team.
• Provide a specialist telephone triage assessment and support for
health/social care professionals and patients and carers.
• Receive appropriate referrals from Palliative Care Triage, facilitating care and support in a responsive manner according to priority of need.
• Following specialist holistic assessment and interpretation of the
information collected, in conjunction with other health care
professionals, patient and the carer, formulate and implement a plan of action. Continually review programme according to changing needs.
