Job Description:
The Clinical Social Worker utilizes clinical expertise to perform psychosocial assessments, develop and implement plans of care appropriate care team, assess crisis situations to appropriately provide needed resources and or referrals for the patient, and to provide family and individual treatment as indicated. The Emergency Department Social Work Care Manager is part of a team providing services, resources, and clinical intervention to a variety of patients and families. Our role in the emergency room is to respond and provide trauma intervention, grief and loss support, general social work and community resourcing, drug and alcohol assessment and placement, care management, utilization initiatives and readmission goals, and mental health/emotional support.
This PRN position supports our emergency departments at Utah Valley Hospital, American Fork Hospital, Riverton Hospital, and Alta View Hospital. Shift times would vary and are available 7 days a week in 6- and 12-hour increments during early morning, day, evening, and overnight hours. All team members have night, weekend, and holiday shift requirements. PRN staff are required to work a minimum of one shift per week.
