HBPC focuses on Veterans in advanced stages of chronic disease, particularly those at high risk of recurrent hospitalization or nursing home placement. The HBPC program provides comprehensive, interdisciplinary primary care services in the homes of Veterans with complex and chronic, disabling disease.

The CLC and HBPC program provide palliative care, rehabilitation, disease management and care coordination services.

Palliative Plus program is a funded strategic initiative and component of the current Palliative and Hospice Care (PHC) program. Palliative Plus focuses on creating a level of support beyond typical Palliative Care focusing on hospice-eligible Veterans who decline hospice enrollment but agree to be followed by the team. This program has been shown to decrease utilization of overall healthcare related costs in the context of providing this level of case management.

Home-Based Memory Care is a funded strategic initiative intended to support aging in place and delay institutional placement by addressing the specific gaps in memory care that often result in placement. The concept of "Aging in Place" is a priority of VHACO-GEC.

TBI and SCI/D clinics are both foundational services of VHA. The SCI/D clinic serves as a spoke site to the hub at the Minneapolis VA HCS.

Responsibilities:

o Comprehensive management of the physical, psychological, emotional, social, and spiritual needs of the Veteran.

o The Associate Medical Director is an integral member of the CLC and HBPC teams and will work with an interdisciplinary team of professionals from medicine, nursing, social work, psychology, chaplaincy and other providers to meet the needs of Veterans and families served by these programs.

Duties include, but are not limited to:

• Assures that each patient presenting for care at the clinic receives quality medical care within the limits and resources of the clinic, obtaining referral to another VA facility if resources are not available.
• Directs and supervises the clinical activities of the physicians, physician assistants, and nurse practitioners. The Chief may also have input on the performance of others such as nurses, administrative support staff, social workers, clinical pharmacy practitioners, and nutritionists.
• Participates in direct patient care and serves as a role model for peers. Consideration is given to the age of the patient as it relates to the ability to understand directions and provides objective self-assessment or any other medical milieu appropriate to the overall treatment plan.
• Ensures that staffing, space, equipment, and materials, systems and support services are utilized appropriately to meet the needs of the patients served in the outpatient clinic.
• Ensures that patients receive care in a safe and therapeutic environment.
• Ensures that the care of each patient in the outpatient clinic is the responsibility of a qualified and appropriately privileged staff physician.
• Is responsible for enforcement of applicable medical center and outpatient clinic policies, rules and regulations, and medical staff bylaws.
• Is responsible for meeting the performance improvement requirements of both the Joint Commission and the Department of Veterans Affairs, encompassing the information in the outpatient clinic, and for directing, educating, and involving all staff in these areas in the performance improvement process.
• Completes annual proficiency evaluations of physicians, PAs, and APNs, and incorporates provider specific performance improvement data in these appraisals.
• Participates in the resource allocation process for the outpatient clinics.
• Has overall responsibility for the safety program and action plan in the outpatient clinic.
• Participates in the various medical center's committees as appropriate.
• Administers and enforces all leave policies for the individuals supervised. Utilizes workload statistics and other data to ensure optimum availability of staff to meet patient needs.
• Anticipates clinic staffing vacancies and needs, interviews and selects candidates.
• Completes annual performance evaluations of physicians and incorporates specific performance improvement data in these appraisals.
• Arranges for physician coverage to assume the clinical responsibilities of the Section Chief during his/her absence.
• Is responsible for the oversight of all educational programs in the clinic and participates in the educational programs for primary care.
• Uses results of monitoring and evaluation activities in the clinic and clinic improvement activities, in planning the educational programs for the medical staff.
• Encourages and supports professional growth of staff by approval of authorized absence for conferences and workshops, in accordance with the prescribed guidelines by Human Resources Management Service and ACOS/Education.

The Robley Rex Veterans Affairs Medical Center (VAMC) improves the health of our veterans by providing a continuum of care through an integrated health care delivery system. Duties include but are not limited to:

- Monitoring and ensuring that Specialty Care Service complies with agency regulations, medical staff bylaws, facility policies, Joint Commission (JC) standards, and other regulations.
- Overseeing professional and administrative activities within the assigned division including selection, orientation and continuing education of staff in collaboration with other Physician leaders.
- Assisting in coordination of program development to strengthen GEC clinical programs and operational issues; and advise the ACOS, GEC; about the adequacy and appropriateness of the organization's scope of services and its professional and support staff.
- Assuring that individuals with clinical privileges competently provide service within the scope of privileges granted.
- Recommending appointment and clinical privileges for each member of the division and others requesting privileges within the division.
- Developing and implementing policies and procedures; oversight of administrative and programmatic resources; and monitoring outcomes using data-driven quality assurance processes.

The Medical Director for Community Care Services (CCS) will have medical oversight and exercise independent judgment in carrying out duties and responsibilities that include but are not limited to programmatic supervision as well as general oversight of care in the community throughout the VA Puget Sound Health Care System (VAPSHCS). The incumbent reports to the Chief of Staff office and will be responsible for the tracking, monitoring, reporting, supervision and oversight of productivity, service reports and special requests for reports.

Responsibilities and duties include but are not limited to the following:

• Provide vision, supervision, and direction to staff and programs in CCS which is comprised of Non-VA care (NVCC), Choice and MISSION Act programs.
• Take prompt actions to assure that staffing in CCS is always adequate to accomplish required tasks and program missions.
• Works collaboratively with Clinical Informatics, Health Administration Service and Clinical Services regarding processes that overlap with CCS programs. Interprets and implements national and VISN guidance for the CCS programs.

• Develops, negotiates, and renews all agreements with non-VA care fee providers, Choice, and Mission Act Providers so that negotiated amounts conform to national VA guidelines and are fiscally responsible with federal government resources.
