Provides comprehensive psychosocial and psychiatric initial assessments as ordered or requested. This position works primarily in the Emergency Department
$3,000 Sign On Bonus*
Pay Range: $29.96 - $49.25/hour
Employment Type: Part Time Nights, 48 hours every 2 weeks
Schedule: 3:00 pm - 3:00 am, includes every 3rd weekend
Additional Requirements: Two holidays per year

Assessment and Treatment:
Provides comprehensive initial psychiatric assessments including formulation and treatment recommendations.
Able to make an accurate DSM diagnosis that is consistent with the presenting problems, history, mental status exam and/or other assessment data.
Demonstrates ability to analyze assessment data and to determine the appropriate level of care needed for clients with mental health or substance abuse problems.
Assesses and provides both treatment and discharge planning services to inpatients who present with mental health or substance abuse problems.
Documentation and Communication:
Effectively and accurately documents assessment data and formulation.
Communicates results both verbally and/or in writing to the appropriate professionals and coordinates care for appropriate care delivery.
Utilization Management:
Reviews patient data in relation to appropriate utilization of resources and makes treatment recommendations based on that assessment.
Collaborates with the patient, family, members of the health care team and payer sources regarding appropriate level of care and/or alternative resources.
Facilitates referrals as needed to assist in recovery from mental or physical illness and to facilitate access to services such as financial assistance and housing.
Determines that insurance authorization is in place for professional and facility billable services.
Trauma Support:
Provides crisis intervention and grief support for patients or family members involved in a traumatic incident.
Coordinates the services provided to family members of trauma patients to allow the medical team to focus on the trauma victim.
Ensures emotional support and assistance to family members.
Provides information and referrals to family members and significant others.
Requirements:
-Graduate of a School of Social Work approved by the Council of Social Work Education with a Master's Degree required
-Two years of post masters level work in a psychiatric setting or a clinical setting that also deals with psychiatric issues required
-Fully licensed Social Worker in the State of Michigan (LMSW) required
-Current BLS for the Healthcare Provider by 60 days after hire required.
-Mental health and substance abuse training and/or certification preferred
-Three years experience in psychiatric clinical work setting that requires assessment, diagnosis and treatment preferred
-Experience with hospital discharge planning, case management, emergency department work preferred

Position Overview

Scope of work
HDVCH Pediatric Autism Clinic Social Worker - LMSW. Plans and prioritizes care for individuals and population of clients, focusing on strategies that will promote optimal health within populations. Provides peer support and role modeling related to the development of life skills and health/wellness. Demonstrates expertise, current knowledge in patient care and management of a caseload of clients of varying complexity and seeks to improve patient, family and health systems/community outcomes through the application of educational concepts/skills and preventive care in a managed care environment.Assesses internal and external referrals to identify patient/significant others’ needs, level of intensity, insurance benefits and other patient resources.
